Below you will find data sheets, forms, etc, that you can use to begin, or improve, data collection at your location.  Many of these forms will help you with improving your data collection and understanding of a person’s behavior, and/or will help you work more effectively with your consultants as you try to address behavioral issues.

ABC Sheet - This form can be used for completing a descriptive analysis.  Parents or staff record incidents of a defined behavior, along with immediate antecedents and consequences along with record date and time.  There are many versions of these, but this is the version FCBD uses most often.  This is a PDF File, and will open with Adobe Acrobat.

Scatterplot - Scatterplots are used to record occurrences across time.  Once several days or weeks of data are gathered, it can be analyzed for patterns in times of day, day of the week, and so on.  This sample is a 31-day, half-hour scatterplot, but they can be made to record more or less frequently as well.  This is a PDF File, and will open with Adobe Acrobat.

Time Sample - Similar to a scatterplot, but used for a slightly different purpose - to record behavior that occurs in too high a frequency to adequately measure on an ABC Sheet.  Parents or staff record a defined behavior at a predetermined frequency - e.g. whether the behavior occurred or not within a given time frame. This sample is a 31-day, half-hour time sample.  This is a PDF File, and will open with Adobe Acrobat.

Shift Rating Sheet - While not ideal, a shift rating sheet can be useful in getting objective information in situations where it is difficult to get more accurate data.  Parents or staff rate a defined behavior on a 0-3 scale.  This sample focuses on “agitation”, and covers three shifts over 31 days.  This is a PDF File, and will open with Adobe Acrobat.

Blank Data Sheets.xls - All of the data sheets listed above, plus completed samples for the bottom three sheets, in one file.  This is a Microsoft Excel spreadsheet file - it will require Microsoft Excel to open.  This is the option you want if you have Microsoft Excel, and you want to edit the forms on your computer. 

Advanced ABC Sheet - This ABC Sheet is designed for the experienced user who has use for multiple avenues of data.  It provides location codes, severity ratings, and a means for tracking non-target or emerging behaviors.  This is a PDF File, and will open with Adobe Acrobat.
