Erin Wade, Ph.D. -Dr. Wade has a Ph.D. in psychology from Marquette University, and is a Board Certified Behavior Analyst (BCBA). He has many years of experience as a behavioral consultant for adults and children with developmental disabilities. Dr. Wade has experience using the techniques of Applied Behavior Analysis (ABA) in multiple settings, from single family homes to schools and large facilities. Dr. Wade is approved to provide behavioral consultation services in Illinois through the Adult DD CILA Waiver and through the Children’s Residential and In-Home Services Waivers.
Annette Miller-Crone, LCSW - Ms. Miller-Crone has her master's degree in social work from the University of Illinois. She is experienced in working with people with developmental disabilities at all levels of service, from direct care and case management to social work and resource access services. Ms. Miller-Crone is approved to provide counseling services in Illinois through the Adult DD CILA Waiver.
Ted Dunphy, R.Ph. - Mr. Dunphy is a registered pharmacist with many years of experience in reviewing and consulting on the often complicated medication regimens of individuals with developmental disabilities.
Steven Kouris, D.O., M.P.H. - Dr. Kouris brings to the FCBD Team his years of dedicated service in the psychiatric treatment of children and adults with developmental disabilities.
Aleta Woods, M.Ed., BCBA - Ms. Woods has a master’s degree in early childhood special education and is a Board Certified Behavior Analyst. She has experience as a classroom teacher and in using ABA in school and home settings. Ms. Woods is approved to provide behavioral consultation services in Illinois through the Adult DD CILA Waiver and through the Children’s Residential and In-Home Services Waivers.
Patsy Cook, M.S. - Ms. Cook has a master’s degree in clinical psychology from the Harold Abel School of Psychology at Capella University. She has extensive experience working with children and adults with developmental disabilities in multiple areas, including program development for early intervention, adolescent, and adult recreational, residential, and vocational services. Ms. Cook is trained in and experienced with the specialized intellectual assessment needs of people with developmental disabilities.
Marnie Wade, Business Manager - Ms. Wade has a bachelor’s degree in communications from Western Illinois University. She has worked at multiple levels of service delivery for people with developmental disabilities, including as a direct service provider, home supervisor, QMRP and Residential Services Coordinator, Human Services Employment Manager, and most recently as the Training Coordinator for Milestone, Inc. in Rockford, IL. She brings this wealth of experience to the Forest City to aid us in coordinating and managing services.
